In Order for Health Insurance to approve Durable Medical Equipment, please meet the following criteria to qualify, which must be documented in the beneficiary medical record.

Please specify BRACE instead of SUPPORT on prescriptions, including in progress notes and medical summary. In the progress notes plan, it must state the items you are prescribing.
